How many signers are there?
Signer 1: What type of identification will you physically present to the notary at your appointment? We highly recommend you use a driver's license or passport.

We generally have a zoom review of documents at the end of the appointment so that we can ensure they have been completed accurately. This takes about 10 minutes. Do you agree to having this review?

Please answer the following questions to help us know how best to prepare for the appointment and help you have peace of mind.

Are you open to providing a digital copy of your ID ahead of time in order to minimize contact when you meet them in person? (You can just hold it in front of them for verification when you meet them in person)
Would you prefer to meet with the notary outside of your residence or office (for example, on the street or in an open area?)
Are you vaccinated?
Do you have a preference on the vaccination status for the notary?
How chatty/friendly would you like your notary to be?
I understand that there is risk involved in meeting with a notary despite the company's best effort in screening. I agree to take all the necessary precautions to minimize the risk of COVID-19 infection. I also agree to update the company if there are any changes in my health situation after signing this agreement.
